Replaced short orginal intro and link to the dated developer's section template example with step by step instructions as discussed in the devel mailing list. I have reviewed the steps and checked the text but please review and make any changes/corrections as needed.
I just had an issue installing anti-evil-maid and couldn't find documentation on how to remove it. Looking at the source I think this is the expected way (which worked for me).

This commit creates a single, central page (/doc/usb/) for all
USB-related information by combining stick-mounting.md with the
manual USB qube (formerly known as "USBVM") creation procedure
from security-guidelines.md. It rewrites the content from both of
those and adds the content requested in QubesOS/qubes-issues#1690.
It also redirects /doc/usbvm/ away from the ancient page on PVUSB.
Explanation: All of our other pages consist of an .md file with
the name (or a suitably similar name) of that page. This commit
renames `configuration/salt/index.md` to `configuration/salt.md`
so that it will follow the organization of the other pages.
