Add 4.0 content to usage of qubes-usb-proxy
Also set "What if I removed the device before detaching it from the VM?" to R3.2 because procedure should be unnecessary under 4.0 and attempting it on 4.0 results in "libxl: error: libxl.c:2233:device_disk_add: device already exists in xenstore".
